IN THIS PART OF the world, sexual health is a dreaded topic publicly.

We are all virgins and pious humans out there, meanwhile, inside the comfort of our bedrooms, we bang more than knock-outs and dynamites put together.

Everyone is either too religious or too scared to openly talk about matters that disturb their sexual and reproductive health, especially ladies.

As such, the problems keep soaring. Sadly.

We no wan gri say we like fuck! Even some married people dey shame to talk say dem dey fuck!

INTRODUCTION
Recently, I saw a case of a young 24-year-old recently-married woman who had a ruptured tubal ectopic pregnancy and nearly died.

What it means is that she got pregnant and the baby was implanted outside the womb (in the Fallopian tube in this case), and the baby grew in the tube until it burst, resulting in serious bleeding and loss of consciousness.

Weeks after she survived the scary episode, she could not come to terms with the fact that she now had one Fallopian tube, which could even have issues if care wasn’t taken (because the burst one was removed during the life-saving operation).

A few minutes of conversation with her and I realized that she’d had untreated Pelvic Inflammatory Disease (PID) a few months earlier. She actually saw the symptoms, but didn’t think it warranted seeing a doctor, because, according to her, they were not severe enough.

Regardless, she kept poking me to find out why she had to be so unlucky as to have the ectopic pregnancy when she was already married and now needed a child.

But with her history of PID in mind, it would be so easy for any doctor to understand why she was at an increased risk of having an ectopic pregnancy.

PID is one of the easily-overlooked causes of fertility challenges that sexually active women have in this part of the world.

DEFINITION
In simple terms, PID – Pelvic Inflammatory Disease – is an ascending infection of the female genital tract. It arises when bacteria that enter the vagina cause infection that continues to go and grow upwards, from the cervix (the gateway of the womb) to the womb itself, the Fallopian tubes, and even beyond.

CAUSAL ORGANISMS
The most common organisms causing it are
Chlamydia trachomatis and Neisseria gonorrheae. There are a host of others too, example the Bacteroides spp.

CLINICAL FEATURES
The infection usually presents as vaginal discharge, low abdominal pains, fever, and sometimes in addition to nausea and vomiting or foul-smelling vaginal discharge. On pelvic examination, there could be pain when the examining finger touches the cervix, known as cervical excitation tenderness.

In some ladies, PID can manifest as painful menses (secondary dysmenorrhea) or pain while having sex (dyspareunia).

RISK FACTORS
PID is traditionally commoner among sexually active young ladies, usually less than 25 years who have multiple sexual partners, or who often practice unprotected sex.

We know that some babes love dick so much, they know all forms of sizes and shapes of dick that they can describe it all for you very easily. These babes are supposed to be called DICK-TIONARY. 🤣🤣🤣

Other risk factors are: regularly douching after sex, or previous history of PID.

COMPLICATIONS
The reasons why this PID is so dreaded are plenty, more especially because if left untreated or poorly treated, it can affect the woman’s reproductive life afterwards.

The main complications of PID are:
~ Ectopic Pregnancy: PID is notorious for causing tubal ectopic pregnancy cos the scar tissue formed after the PID prevents the ovum from migrating down the tubes.
~ Infertility: PID can damage reproductive organs in women, especially the tubes, making pregnancy extremely difficult.
~ Tubo-ovarian abscess: Here, an abscess forms and accumulates in the Fallopian tubes and on the ovaries.

PREVENTION
PID can be prevented, but some ladies no wan hear. For this to be achieved, sexually active ladies should:
~ Avoid multiple sexual partners
~ Practice safe sex
~ Avoid douching
~ Always see a DOCTOR quickly as soon as you experience any of the symptoms.

TREATMENT
Usually, because PID is most often a clinical diagnosis for doctors, we usually commence treatment even before the microbial culture and sensitivity results are out.

The preferred drugs are usually antibiotics.

CONCLUSION
So when next you are banging upandan and you start to have any of these symptoms, remember that you’re about to play with fire.
